The idea of “brain foods” often gets flattened into simple lists: eat this berry, this fish, this handful of nuts, and your memory improves. The research the idea is loosely based on is more specific and more interesting than that. In 2015, nutritional epidemiologist Martha Clare Morris and colleagues at Rush University Medical Center developed what they called the MIND diet, a hybrid of the Mediterranean and DASH diets built around ten brain-healthy food groups, including leafy greens, other vegetables, berries, nuts, olive oil, whole grains, fish, poultry, and beans, while limiting red meat, butter, cheese, fried food, and sweets. Morris’s team followed 923 older adults for an average of 4.5 years and found that people who most closely followed the MIND diet had up to a 53 percent lower risk of developing Alzheimer’s disease compared with those who followed it least closely, with a smaller but still meaningful reduction, around 35 percent, for people who followed it only moderately well. A related paper from the same research group, tracking nearly 1,000 participants for closer to 4.7 years, found that closely following the diet was associated with a rate of cognitive decline equivalent to being roughly 7.5 years younger than people who followed it least.
Those are striking numbers, and they’re the reason the MIND diet spread quickly from a research finding into general health advice. But it’s worth being precise about what kind of evidence produced them. Morris’s original studies were observational: researchers tracked what people already ate and compared their outcomes over time, rather than randomly assigning some people to eat one way and others another. Observational research like this is genuinely useful, and hard to dismiss when the pattern is this strong and consistent. But it can’t fully rule out the possibility that something else about people who eat this way, more education, more money, more consistent healthcare, more exercise, is doing some or all of the real work, even after researchers statistically adjust for the factors they can measure.
That’s exactly the kind of question a randomized controlled trial is designed to answer, and in 2023 one was finally run. Researchers published a trial in the New England Journal of Medicine that randomly assigned 604 older adults who were overweight, had a family history of Alzheimer’s, and already ate a less healthy diet, to either follow the MIND diet or follow a control diet, with both groups also getting mild calorie-restriction counseling. After three years, the two groups showed no significant difference in cognitive function or in brain imaging measures of neurodegeneration. Both groups improved cognitively over the study period, likely reflecting the general benefit of nutritional counseling and modest weight loss, but the MIND-specific diet pattern didn’t outperform the control diet the way the earlier observational research had suggested it might.
That result doesn’t erase the earlier findings, but it meaningfully changes how confidently they should be stated. It’s possible the MIND diet’s real-world benefit depends on following it for far longer than three years, since Alzheimer’s-related brain changes unfold over decades, and dietary trials rarely run that long. It’s possible the benefit shows up more clearly earlier in life, before the kind of risk factors this trial’s participants already had. It’s also possible that some of the original observational association really was explained by other differences between people who eat this way and people who don’t. The honest state of the evidence right now is that a specific, well-designed test of the MIND diet’s short-term effect on cognition came back null, even though the food groups involved, vegetables, berries, fish, nuts, olive oil, remain well supported by decades of broader cardiovascular and metabolic health research for reasons that have nothing to do with memory specifically.
None of this means diet is irrelevant to brain health, or that the foods on the MIND diet’s list are a bad idea. Diets rich in vegetables, fish, and unsaturated fats, and lower in red meat, fried food, and added sugar, have a long, separately well-supported track record for cardiovascular health, and cardiovascular health is itself closely tied to brain health, since the brain depends on healthy blood vessels to function well as people age. What the current evidence doesn’t support is treating specific “brain foods” as something close to a proven memory intervention with an effect size researchers can currently guarantee. The pattern connecting this kind of diet to a lower rate of cognitive decline is real and worth taking seriously. Whether eating this way, starting today, meaningfully changes an individual’s cognitive trajectory is a harder, still-open question, and the most rigorous test of it so far didn’t find a clear yes.
